Medical Case Management

Medical Case Managers work with adults living with HIV (ages 25 and older) to ensure coordination of medical and supportive services. This includes referrals for:

  • medical care
  • medication assistance
  • housing
  • food and pantry vouchers
  • mental health
  • substance use
  • pastoral care
  • other identified needs

Typically Case Managers are the greatest source of support for clients and their loved ones while they are learning to live with HIV. Education for both the client and their family is very important to a client’s well-being, sense of worth, and to help combat stigma.

Clients depend on their Case Managers to assist with obtaining community resources and maintaining good health. Health care providers also depend on Case Managers to assist clients with obtaining positive health outcomes, breaking barriers to effective treatment, and bridging the gap in communication between the client and the healthcare system.
