Medical CAse Management

Case Managers work with HIV+ adults, ages 25 and older. Key services include:
- making sure they have a physician
- ensuring they have access to medications
- obtaining housing, substance abuse treatment or mental health services.
Typically Case Managers are the largest source of support for clients and their loved ones while they are learning to live with a stigmatized disease. Education for both the client and their families is very important to a client’s well-being and sense of worth.
Clients depend on their Case Managers to assist with obtaining community resources and maintaining good health. Health care providers also depend on Case Managers to assist the clients with obtaining positive health outcomes and breaking barriers to effective treatment.
